Team Entry information

Team Rules

Team entries must be completed by the coach.
Coaches simply type a list of each contestant and their contest age, including brithdate for each team.
List the type of team- Dance Twirl, Show Twirl, Twirl, Pom pon, Danceline or Cheerleading.
Fees are $6.00 per contestant.

You may e-mail your list to Ann: ann@annsallstars.org. Then mail payment and waivers to address below.

There is no processing fee added to contestants who enter only team events.
Please include a signed waiver for each member.
Include coaches name, phone and e-mail.

Make Checks payable to Ann Durkin and mail the entry along with payment to:
