Parent or guardian accepts full responsibility for twirlers participation in this event. I agree to hold NBTA, Contest directors and Evangelical Covenant Church harmless in the event my child is injured while participating. Signature is required for the contestant to participate.

Payment and waiver must be postmarked by October 4, 2008 or you will be charged a $20.00 late fee. Please include late fee if mailing after October 4, 2008. Any payment not received by October 15, will result in contestant being dropped from contest unless prior arrangements have been made.Mail to:
