Parent or guardian accepts full responsibility for twirlers participation in this event. I agree to hold NBTA, Contest directors and Evangelical Covenant Gym harmless in the event my child or any member of my family is injured while participating. Payment and waiver must be postmarked by Sept. 13, 2010 or you will be charged a $20.00 late fee. Please include late fee if mailing after Sept. 13, 2010. Any payment not received by Sept. 22nd, will result in contestant being dropped from contest unless prior arrangements have been made.Mail to:
